WELCOME TO PALM VALLEY ORAL SURGERY

Our office functions quite uniquely from a general dentist practice so please review carefully.

Before you come to our office

  • Your dental office will refer you to our office. Your dentist may make your appointment for you OR you can call us and schedule your appointment. When you telephone our office, you will be greeted by one of our Scheduling Coordinators who will ask you what the appointment is for and who referred you. If you have dental insurance, please have the card available as we will ask you for this information.
  • Please PRE-REGISTER with our office by downloading our on-line registration forms. Complete these forms in their entirety and either mail the form to 1646 N. Litchfield Rd Suite 130 Goodyear AZ 85395 or bring with you at your initial appointment.
  • Your dental office will give you a referral slip; please bring this with you on your initial visit. If your dentist has a current x-ray of your mouth bring this with you as well. Please keep in mind that if you do not have your current x-ray with you at the appointment, we will need to take a new one.

When you get to our office for your initial visit

  • Bring your insurance card.
  • If you are under 18, you MUST be accompanied by your parent(s) or legal guardian.
  • Generally, your first appointment will be for a pre-surgical evaluation. Treatment is usually not done on this day. We will verify your eligibility with your insurance carrier and do our best to estimate your coverage for the proposed treatment.

Your Surgery Day

  • We strive to stay on time. However, emergency patients often require our immediate care and attention. Ultimately, you will receive our full attention as well. Thank you for your patience.
  • You will be asked to pay for the surgery/anesthesia on this day. If we have an estimate of your insurance coverage, you will be asked to pay your portion. Should the insurance company pay more than the estimate you will receive a refund. However, if they pay less than the estimated amount, you will receive a statement of the amount you will owe.
  • The responsible party who will be driving you home MUST REMAIN in our office during your entire procedure. We will cancel and re-schedule your surgery if this person leaves our office for any reason.

After Surgery, Following-up with Our Patients
Our commitment to caring for you goes beyond your surgery. You will receive a courtesy, follow-up telephone call on the evening after your surgery as well as a series of post-operative calls within the initial two-week post-operative period. Our motivated staff is eager to answer all of your questions regarding follow-up care. Please express any questions or concerns you may have during your phone visits.
